Transitions Good Practice Guide
Audience: Early years professionals, parents and other professionals supporting children or young people on the autistic spectrum.
This Transitions Good Practice Guide has been produced by Norfolk’s Autistic Spectrum Disorder (ASD) Steering Group with input from the statutory, voluntary sector and parents.
The pack offers information, planning guidance and record sheets to aid successful transitions for children and young people with autism.
A planning and recording toolkit is provided and should be completed for all significant changes for a child or young person on the autism spectrum. Ideally where a diagnosis has been obtained in the early years, planning needs to start as soon as the child is being considered for any pre-school provision, and then continue through his/her educational career. For others, when diagnosis comes later, this needs to start with immediate effect.
