Community care is the non-medical services provided by social services departments to help anyone over the age of 18 in the community who has additional needs because of a disability such as autism.
Services might include:
- Day care
- Assistance with housing
- Equipment
- Other services to meet your specific needs
Some community care services are also available to children.
In order to access this care, you will need to be assessed by a social worker.
If you are still attending school and have a statement, social services will be involved in your transition planning and drawing up a care plan.
Some local authorities give the option of direct payments so that the individual has more choice in the type of support they need.
